Initials on DCS & including e.g. text

I received the following questions from a district today. Although the
answers seem clear to me, to be on the safe side... we would like to
hear them from you. Answers are in CAPS.

1. Staff initials on DCS - teacher is indicating that profile is
"printing them" this year, whereas last year it did not. They want to
know if the teacher still needs to personally sign or initial the DCS,
or is the preprinted name and initials sufficient? RESPONSE: ON A DCS
THERE IS THE "STAFF RECORDING DATA FOR EACH DATE (REQ'D)" SPACE AND THE
"NAME AND INITIALS OF STAFF RECORDING DATA (REQUIRED)" SPACE; IT IS
ACCEPTABLE TO COMPLETE THESE SECTIONS IN PROFILE. THE TEACHER OR
PERSON(S) IN THE STAFF KEY SECTION DO NOT NEED TO ALSO PERSONALLY SIGN
THEIR NAME AND INITIALS IN ADDITION TO IT BEING COMPLETED IN PROFILE.
2. SAT - is it okay to omit the "eg" portions that are in SATs when
transcribing them as the task? (the examples that are generally at the
end in parenthesis)RESPONSE: IT IS ACCEPTABLE FOR TEACHERS TO NOT
INCLUDE THE "E.G." INCLUDED WITH SOME SAMPLE ASSESSMENT TASKS. IN MANY
CASES THE "E.G." IS INCLUDED TO PROVIDE INSTRUCTIONAL IDEAS REGARDING
THE SAMPLE ASSESSMENT TASK. ALSO, REMIND TEACHERS THAT SAMPLE ASSESSMENT
TASKS MAY BE MODIFIED AS APPROPRIATE FOR THE STUDENT.
