Thanks for your question about what to do with the Consent to Photograph (both Student and Peer) forms.
These forms must be sent to the student's district of residence where they are maintained in the student's file in the district's Special Education office. It is a local decision regarding how the papers are sent to the district of residence by the BOCES.
Please note that teachers are not allowed to place a consent form in a datafolio. This is because the datafolio can be discarded after one year or given to a parent and the consent forms would be lost.
