Parent Forms for Residential Students

Alicia,
We have students living in a residential facility. Are their cottage
staff allowed to fill out the parent survey or do the parents have to do
it? Parents may not have an idea about the student's ability.

Thanks for your question about the Parent/Family/Guardian Survey.
The guidance from the Department in past years has been that the
student's parent/family or guardian must complete the survey.
Cottage/residential staff may not. If the teacher is unable to complete
the survey with parent/guardian input then they should record the
attempts on the documentation pate as indicated. Thanks much!
